Canada’s Accessible Transportation for Persons with Disabilities (ATPDR)

For travelers to and from Canada only, we let travelers save their accessibility services information with us for easy access later. Learn more about the ATPDR.

Customers traveling from Canada without vaccination from COVID-19

For travelers departing from Canada who are not vaccinated against COVID-19, a signed medical exemption form must be submitted to United for verification at least 72 hours prior to your travel date. Download the medical exemption form

Once the signed form is complete, please submit to canadarequirement@united.com
