PetSafe Booking Request Form

To request a booking for your pet, please complete the form below. Please note that this form is only applicable for travel originating in the U.S., Canada or Guam (excludes travel from Puerto Rico or the U.S. Virgin Islands). If you need to schedule your pet for travel originating elsewhere, please contact your local pet shipper.

Within three business days after your form is submitted, a PetSafe® representative will contact you about your booking request.
