Membership forms
The fastest way to apply for or renew a Red Carpet Club membership is to join or renew online. If you do not want to submit your application online, you can instead download, complete and mail in the Red Carpet Club application (pdf).
Whether you are applying for membership online or by mail, if you and your spouse or domestic partner have different surnames, you must complete and submit by mail an affidavit of marriage/domestic partnership (pdf).
