Mileage Plus Choices Program Terms and Conditions

• Miles accrued with your Mileage Plus credit card for eligible card activity posted to your credit card statement beginning with your January 2006 statement are available for Mileage Plus Choices program redemption. Miles accrued for credit card purchases, enrollment, Chase promotional offers, anniversary renewals, and other eligible card activity are available for redemption. Miles earned for Chase non-credit card activity including, for example, debit/check card purchases or home equity advances are not eligible. Miles earned for activity that is rewarded by third parties as bonuses associated with use of the Mileage Plus credit card also are not eligible. Your Mileage Plus Choices program balance can be found by visiting www.united.com/chase and logging in. You must have previously earned miles with your Mileage Plus credit card and be a current cardmember to be able to login.
• Miles accrued with your Mileage Plus credit card will continue to be reflected in your Redeemable Miles balance. Any Mileage Plus Choices program redemption activity that uses miles will result in an equivalent reduction of your Redeemable Miles balance. Also, any traditional Mileage Plus redemption that exceeds your Redeemable Miles balance and requires the use of miles earned with your Mileage Plus credit card to complete the redemption, will result in a concurrent reduction in your Mileage Plus Choices program balance.
• The Mileage Plus number associated with the Mileage Plus Choices program balance being redeemed must match the Mileage Plus number associated with the credit card activity being redeemed against. You cannot redeem your miles for a statement credit on another person's Mileage Plus credit card.
• The accrual of miles and the redemption of awards are subject to the rules of the United Mileage Plus program. Program rules are subject to change without notice. Please allow 6 to 8 weeks after completed qualifying activity for miles to be posted to your Mileage Plus account. Miles may not expire as long as you earn or redeem miles at least once within 18 months. Activity includes flying, using your Mileage Plus Visa and more.

Choices Program Air Redemption Terms and Conditions

• To redeem, purchase any airline ticket on united.com with your Mileage Plus credit card. Flights purchased through any other source, such as United General Reservations, United Vacations, travel agencies or other travel websites, are not eligible. When the purchase posts to your credit card account, login at www.united.com/chase to redeem miles earned with your Mileage Plus credit card for a statement credit against that charge.
• Since miles earned with your Mileage Plus credit card are reflected in your Redeemable Miles balance, any redemption activity that uses these miles will result in an equivalent reduction in the your Redeemable Miles balance.
• No blackouts means any flight available on united.com can be purchased for Mileage Plus Choices program redemption any day of the year. No inventory restriction indicates that any open seat on any available flight can be purchased on united.com for Mileage Plus Choices program redemption. Miles earned with your Mileage Plus credit card can then be redeemed against that purchase for a statement credit up to the full cost of the ticket.
• Statement credit request must not exceed the purchase charge, partial statement credits are allowed with a minimum of 5,000 miles. However, only one statement credit request can be made per unique transaction.
• Cardmember must submit redemption request within 90 days of airline ticket purchase date.
• Miles are deducted at time of redemption request. Please allow 3 - 5 days for credit to appear on your account. Cardmember is responsible for making their monthly credit card payment according to the terms and conditions of their account to avoid any late fees or finance charges. Mileage Plus Choices program statement credits are not considered payments unless they cover your entire outstanding balance and post before your payment due date.
• For rejected requests, the reason for the rejection will be emailed to the cardmember and miles initially deducted will be reversed.
• The Mileage Plus number associated with the Mileage Plus Choices program balance being redeemed must match the Mileage Plus number associated with the credit card activity being redeemed against. You cannot redeem your miles for a statement credit on another person's Mileage Plus credit card.
• Airline ticket changes, cancellations and refunds subject to the rules of the airline ticket.
• Miles redeemed in the Mileage Plus Choices program cannot be returned or re-deposited by the cardmember.
• Mileage Plus and Chase reserve the right to reverse Mileage Plus Choices Program Air Redemptions where the original ticket purchase is refunded. Miles for the redemption will be re-deposited in your account and the Choices Program Air Redemption statement credit will be reversed on your Mileage Plus credit card account. A redemption fee of 2,500 Miles may be deducted from your Mileage Plus account and appear on your statement as an adjustment.
• Only miles earned with the Mileage Plus credit card are available for Mileage Plus Choices Program redemptions. The number of miles required for redemption depends on the cost of the airline ticket including all taxes and fees and the amount of the statement credit requested. Cardmember is responsible for all associated taxes, airport security charges, fuel surcharges and other usage fees required for travel. These charges will be included in the overall calculation to determine the number of miles required for redemption. The number of miles required for redemption is subject to change at any time without notice.

Choices Program Hotel and Car Redemption Terms and Conditions

• To redeem, make your hotel accommodation on www.united.com/hotels or your car rental accommodation on united.com/cars with your Mileage Plus credit card. (Hotels or car rentals booked through United Vacations or United Escapes are not currently eligible for Choices redemption) Pay with your Mileage Plus credit card. When the purchase posts to your credit card account, visit www.united.com/chase and login to redeem miles earned with your Mileage Plus credit card for a statement credit against that charge.
• Since miles earned with your Mileage Plus credit card are reflected in your Redeemable Miles balance, any redemption activity that uses these miles will result in an equivalent reduction in your Redeemable Miles balance.
• No blackouts means any hotel or car rental available on united.com can be purchased for Mileage Plus Choices program redemption any day of the year. Miles earned on your Mileage Plus credit card can then be redeemed against that purchase for a statement credit up to the full cost of the purchase.
• For prepaid hotel stays, changes, cancellations and refunds subject to the rules stated on united.com. For non-prepaid hotel stays and car rentals, changes, cancellations and refunds subject to the rules of the individual hotel or car rental company.

Choices Program the Economy Plus annual option Redemption Terms and Conditions

• Redemption: Miles earned with the Mileage Plus credit card may be redeemed for the Economy Plus annual option. The number of miles required for the Economy Plus annual option is subject to change. Miles redeemed for the Economy Plus annual option are not refundable.
• Since miles earned with your Mileage Plus credit card are reflected in your Redeemable Miles balance, any redemption activity that uses these miles will result in an equivalent reduction of your Redeemable Miles balance.
• Membership and use: the Economy Plus annual option is a fee-based membership program that allows a Mileage Plus general member and one guest to sit in Economy Plus on all United and United Express (where available) flights. Benefits are provided for the exclusive use of you, as the Member, and are nontransferable. Seat assignments in Economy Plus can be requested and confirmed (when available) at the time of ticketing, regardless of ticketing channel. Seating in Economy Plus is not guaranteed, but may be requested and confirmed any time prior to the flight. Guest must be on the same reservation record as the Economy Plus annual option member. Memberships may take up to 48 hours to activate. Membership is valid through the end of the twelfth full month after enrollment. Upon advance notice to the Member, the Membership Fee or renewal terms may change from time to time with 60 days notice.
• Termination: United Airlines has the right to terminate the benefits of this program at any time. Your membership may also be terminated at any time if you violate any of the program rules. United may refund to you the pro-rata amount of the miles redeemed for the unexpired portion of the then-current term.

Choices Program EQM/EQS Redemption Terms and Conditions

• Redemption: Miles earned with the Mileage Plus credit card may be redeemed for Elite Qualifying Miles and Elite Qualifying Segments in increments of 10,000 miles for 1,000 EQM and 1 EQS. A maximum of 50,000 miles may be redeemed for EQM and EQS per year. The number of miles required for EQM and EQS is subject to change. Miles redeemed for EQM and EQS are not refundable.
• To redeem miles earned with Mileage Plus Visa towards 2009 elite status, miles must be redeemed by 11:59pm CST on December 30, 2008.
• Since miles earned with the Mileage Plus credit card are reflected in your Redeemable Miles balance, any redemption activity that uses these miles will result in an equivalent reduction of your Redeemable Miles balance.
• Additional Terms: EQM/EQS only count toward elite status and do not count toward award redemption. In the event that your status is upgraded, it will be reflected in the system within five days. Mileage Plus elite status earned based on EQM or EQS accumulated in 2008, including EQM/S through Mileage Plus Choices program redemption, is valid through January 2010. Redemption of EQM/EQS through this program does not guarantee elite status. Mileage Plus elite qualification is required each year based on the number of EQM or EQS earned between January 1 and December 31 of the prior year. Visit united.com/elite for qualification details.
