If you have not received your baggage within 72 hours, download and print United's Baggage Tracing/Claim Form (2MB).
Please complete this form and mail it to United Airlines along with the following:
- A copy of your delayed baggage report
- Passenger ticket receipt
- Receipts for items valued over $250.00
