If you have not received your baggage within 72 hours, download and print United's
Baggage Tracing/Claim Form.

Please complete this form and mail it to United Airlines along with the following:
  • Passenger ticket receipt(s) for each person involved in the loss
  • Claim check(s) or a copy of the delayed baggage report for the missing property
  • Excess baggage charge receipt, if applicable
  • Excess valuation receipt, if applicable
  • Purchase receipt(s) or proof of ownership for articles more than $100
  • Copy of driver’s license or other government-issued photo ID
