Lettuce Holiday Bonus Certificates and Frequent Diner Club Points are not awarded for Redemption of United Miles for Lettuce Entertain You Gift Cards.

Dining Gift Cards are issued by Lettuce Entertain You Enterprises and may be redeemed solely at participating Lettuce Entertain You restaurants. Purchases on the Lettuce Entertain You Dining cards will be deducted from the card until the value reaches zero. The card cannot be redeemed for cash except where required by law and cannot be replaced if lost or stolen. The card is not valid toward Frequent Diner points when used and is not valid for private party or outside catering arrangements. Neither Lettuce Entertain You nor United are responsible for lost or stolen Lettuce Entertain You Dining Cards Quantities of Lettuce Entertain You Dining cards available through this redemption are limited. • The United Mileage Plus FTD Rewards program shall be available to Mileage Plus members through December 31, 2010.

AVIS Certificate Terms and Conditions

• Offer valid for two (2) consecutive weekdays, free of the daily time and mileage charge on a Compact through Intermediate size car available at participating AVIS locations in the U.S., Canada and Puerto Rico.
• Upgraded vehicle cost (up to a fullsize car – group D/E) is the sole responsibility of the user of the Award Voucher.
• Award Voucher user is responsible for making a reservation with AVIS at least 24 hours in advance. Customer must contact Avis directly at 1-800-831-8000. Advise the agent you are using an Award Voucher and mention your AWD and coupon number the 2 weekday award.
• Member is responsible for additional rental days not covered by this Award Voucher.
• Taxes, local government surcharges, vehicle licensing fee not exceeding (US dollars) $1.93/day in California, $1.75/day in MT, $2.72/day in TX, $.55/day in UT, $.50/day in HI, $.72/day in GA and $.39/day in FL, airport concession recovery fee up to 15% at some locations and optional items such as additional driver fee and refueling fee are extra.
• For rentals in Canada: Taxes, local government surcharges, Air Conditioning Excise Tax Recovery fee of (CN Dollars) $0.64 per rental day, Vehicle License Fee not exceeding $0.84/day, Airport Concession Recovery Fee up to 15.61% at some locations and optional items such as additional driver fee and refueling fee are extra.
• No more than five (5) Award Vouchers may be used per rental for a maximum of ten (10) free days. Weekday and weekend certificates may not be combined.

• Lost or stolen Award Vouchers will not be replaced.

Terms and Conditions for National 2 Free Days (Weekend or Weekday) Voucher

• Certificates are valid only in the United States.
• Up to three (3) National weekday certificates per rental.
• One (1) National weekend certificate per rental.
• Valid for the rental of a compact through fullsize 4-door car.
• Original certificate must be presented at counter upon arrival.
• Free Day(s) will be calculated by pro-rating against basic rate of entire rental period. Does not include taxes, governmentally-authorized or imposed surcharges, license and concession recoupment fees, airport facility fees, fuel, additional driver fee, one-way rental charge and optional items, which are extra.
• Renter must meet standard age, driver and credit requirements.
• Blackout dates may apply.
• May not be combined with other discounts or promotions.
• Availability is limited for Free Day certificates.
• Valid only at participating National locations.
• Not valid in San Jose, California or Manhattan, New York.
• Receive a free day for every consecutive rental day up to two days. No credit for unused days.
• Weekend rental requires a Saturday overnight keep.

HertzCertificate Terms and Conditions

• Award valid for a rental of a Midsize car for two (2) consecutive weekdays only at a participating Hertz corporate location in the U.S., Puerto Rico or Canada. Award not valid in the New York Tri-State Metropolitan Area.
• One-day advance reservation is required.
• If an upgrade to a higher car class is chosen, any difference in rate is the responsibility of the renter.
• Car Award rental may not be available at some times in some locations, especially during periods of peak demand. Blackout dates may apply.
• Customer must contact Hertz directly at 1-800-654-2210 and book with Mileage Plus discount number and the applicable IT# appearing on the front of the Award Certificate. Hertz will verify that the location chosen will honor the Award Certificate and that cars are available on the dates preferred. Present this Award Certificate at time of rental to the Hertz rental agent.
• The following charges are not included as part of the Award and must be paid for by the renter: taxes, drop off fees, extra days, applicable charges for excess mileage, California vehicle licensing fees, airport related fees, optional refueling service, optional Collision Damage Waiver (CDW) or Loss/Damage Waiver (LDW), optional Personal Accident Insurance (PAI), Personal Effects Coverage (PEC), Liability Insurance Supplement (LIS) and any additional charges which may apply for drivers under the age of 25.
